The name and permit number of the mine and the name and address of the permittee <br />are: <br />Carbon Junction Mine ( CDRMS permit number C- 1992 -080) <br />Oakridge Energy, Inc. <br />4613 Old Jacksboro Highway <br />Wichita Falls, Texas 76301 telephone (940) 322 -4772 <br />2.04.13 (b). No additional acreage was disturbed in 2013. None of the reclamation activities <br />(revegetation of the sediment control system) undertaken in 2009 entailed additional disturbance. <br />c). Backfilling and final grading of mine disturbance was completed in 2006. Backfilling, final <br />grading and topsoiling of the sediment control system (Pond 001, Pond 002, and ditches) was <br />completed in December 2008. No backfilling and grading occurred in 2013. <br />As required by CDRMS Notice of Violation C- 2013 -005, regrading and stabilizing of segments <br />of the Carbon Junction Canyon reconstruction was undertaken to better direct flow along the <br />channel and stabilize drainage form a natural side channel. <br />d). Topsoil replacement was completed for the mine in 2006. Topsoil replacement on the final <br />regraded areas of the reclaimed sediment control system was completed in December 2008. No <br />topsoiling occurred in 2013. <br />
