Permit Number: M-1978-056 Gq Nn AT1QN Operation Name: Durham Sand&Gravel Pit County: Weld -IrTY Annual Fee Due: $791.00 Anniversary Date: June 1,2014 Permit Acreage: 229.89 Current Bond Amt: $300,000.00 According to C.R.S. 34-32.5-116 or 34-32-116,each year,on the anniversary date of the permit,an operator shall submit the Annual Fee,an Annual Report and Map showing the extent of current disturbances to affected land,required monitoring information,reclamation accomplished to date and during the preceding year, any new disturbance that is anticipated to occur during the upcoming year,any reclamation that will be performed during the upcoming year,the dates for the beginning of active operations,and the date active operations ceased for the year. Information contained in this report will be reviewed by the Division upon receipt and prior to the next compliance inspection of the site. If,while completing this report,you learn that your site is not in compliance with the rules and the act,it is advisable that the issues be rectified promptly to avoid possible enforcement action. SUMMARY NARRATIVE: No changes since last years Annual Report. The 2012 Narrative is as follows: The permit was revised by Amendment in 2003 and by Technical Revision in 2005. This Annual Report updates the original 2003 Amendment map that typifies the remaining fill,site prep,reclamation, and related areas of affected lands at play over the permitted area. Minor changes to the map may occur as adjustments are made to areas of extraction and related support features. For example; if the scalehouse is removed near the completion of extraction,the topography of the extracted scalehouse area will be updated at the time of the release request. Also,the fuel depot shown at the center near the south boundary remains and the area under it have yet to be extracted,even though the map shows the topography as if the fuel depot has already been removed and the area beneath it extracted. Finally,permanent access roads may change with time and are therefore approximated,as their final position will be shown at the time of final reclamation. Oil and gas activities affect and potentially reaffect 4.29±acres,and may fluctuate with time depending upon their activities. These disturbances will be updated at the time of final reclamation. All areas shown outside of the planned/existing permanent access roads as shown were at grade with native soil at the time of the permit revision. Previously unaffected land is shown respective of these areas,and any disturbances in these areas will be revegetated at the time of final reclamation,but are not included in the measurements, as internal disturbances comprise the focus of this report,as follows: Tract A has been extracted and slopes reclaimed(graded, soiled and revegetated)as indicated in prior annual reports. Extraction still occurs within the pit bottom and will continue until salvageable materials are exhausted. Tract C is an area of active extraction,principally within the pit bottom and will continue until salvageable materials are exhausted. Approximately 10.07f acres are used for processing and stockpiling of extracted materials,and may shift slightly to accommodate continued extraction within Tract C. The ponded area north of the stockpile/processing area must be drained before it can be considered for additional extraction,as the area is one of the oldest extracted areas of the permit and may have some additional resources remaining to be conserved and recovered for processing. Tract B is an area of active extraction,and the southern portion comprises approximately 9.261 acres as it progresses to the northern portion,presently occupied by apx. 4.48f acres of processing and stockpiling. Here again,extraction will focus over the next year within the existing extraction area bottom and is not expected to significantly progress into the stockpile/processing area over the next year. The balance of lands backfilled,graded, soiled and seeded is reflected in the annual report chart,below. Generally,with the exception of active extraction in Tract B,and minor areas tied to the stockpile and processing activities in Tract C,or active fill activity in Tract D,the majority of affected basin slopes have been graded and soiled, and to a slightly smaller extent, seeded as reflected in the required information,below.
