Exact location of instrument and the date, time and its distance from the blast; <br />iii. Name of the person taking the reading; <br />iv. Name of the person and firm analyzing the seismographic record; <br />V. The vibration level recorded. <br />6.3.4 Exhibit D — Reclamation Plan <br />5. The Applicant has stated that portions of the reclaimed site may receive fertilizer treatment. <br />Please commit to providing the Division with the results of any soil analyses that are <br />conducted prior to applying fertilizers. <br />6. The Applicant has stated that some roads may remain after final reclamation in order to <br />access the reclaimed site. Please specify which roads will remain and identify them of the <br />Reclamation Plan Map. <br />7. The Reclamation Plan Map shows the outlet channel will have erosion control features. <br />Please specify the types of erosion control features to be used and indicate if the outlet <br />channel will be armored. <br />8. Please demonstrate that the 48" culvert in the outlet channel is adequate to convey surface <br />runoff from a 100 year /6 hour storm event; provide any calculations used to size the outlet <br />channel and culvert. <br />6.3.5 Exhibit E — Map <br />9. The Applicant submitted a map labeled as Figure E -1: Sandstone Quarry Existing <br />Topography. Please clarify is this map was intended to serve as the Mining Plan Map. Either <br />revise Figure E -1 or provide a separate map which includes the following elements: <br />a) Signature of the person who prepared the map. <br />b) Label the names of owner(s) of record of the surface area and of the land within two <br />hundred feet of the affected area, identify the owner of the substance to be mined, and the <br />type of structure and owners of record of any permanent man -made structure within two <br />hundred feet of the affected area. <br />
