The Reclamation Plan Map must be revised to include the following elements: <br />a) The name of the Applicant <br />b) The map must be signed by the person who prepared it <br />c) Give the date prepared <br />d) Label the permit boundary <br />e) The map must include a scale. Alternately, the Applicant can specify the length of each <br />side of the permit boundary. <br />f) State the average thickness of replaced overburden; you may make a note on the map that <br />states how much overburden will be replaced, i.e. between 6 to 8 feet of overburden will <br />be replaced. <br />6.3.6 Exhibit F — List of Other Permits and Licenses Required <br />10. The Applicant has stated a Plan of Operations (POO) will be obtained from the US Forest <br />Service.
