The #1 Cutout Line is designated as <br />an environmental protection facility in Henderson's Environmental Protection Plan. The leak was <br />discovered at 8:00am. Originally the leak was believed to have started minutes before the time of <br />discovery, but further investigation indicated there had been a slow leak on the underside of the pipe <br />before the failure occurred. When the failure occurred on May 5th an estimated 500 -100 gallons of tailing <br />slurry escaped. <br />(a) After discovering the issue, Henderson employees immediately stopped flow to the #1 <br />Cutout Line stopping flow out of the leaking line. <br />(b) Mill tailing slurry flow did not adversely impact human health, property or the <br />environment. The entirety of the slurry that leaked from the #1 Cutout Line made its way <br />through the drainage to the tailings impoundment. The drainage in this area filters directly <br />into the tailings impoundment. <br />(c) The contact person for this event is: <br />Amber Moran <br />19302 County Road 3, <br />Parshall, CO 80468 <br />Phone: 720 - 942 -3480 <br />(d) No monitoring or analyses were performed for this situation since Mill water was not <br />released off -site and did not impact any natural water body. <br />(e) The investigation following the incident determined that the leak came from a broken <br />piece of HDPE pipe.
