5. A statement of relevant facts underlying the decision being appealed; <br />6. A discussion of issues raised by the decision being appealed, including identification of <br />any laws, regulations, or policies that were allegedly violated in reaching the decision <br />being appealed; <br />7. A statement as to whether and how the appellant has attempted to resolve the issues <br />under appeal with the Responsible Official and the date and the outcome of those efforts; <br />8. A statement of relief sought; <br />9. Any documents and other information upon which the appellant relies; and <br />10. The appellant's signature and the date. <br />Your appeal must be filed with the Appeal Deciding Officer v ithin 45 days from the date of this <br />letter; or by March 23, 2014. Appeals should be sent to: 111)Pett1 Deciding Officer, USD,4 Foresi <br />Service, Rock -V Mountain Region, 740 Simms, Golden, CO 80401. FAX (363) 275 -5154. <br />Appeals may be filed electronically to meals- rocky - mountain- regional- officeafs.fed.us. The <br />appeal must have an identifiable naive attached or verification of identity will be required. A <br />scanned signature may serve as verification on electronic appeal. <br />Parties may request an oral presentation with the appeal deciding officer under 214.16. The <br />decision shall be implemented unless the Deciding Officer grants a stay under 214.13(b) or an <br />automatic stay goes into effect under 214.3(c).
