PERMIT #: M -1990 -144 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 4, 2014 <br />1. Problem Cited: Boundary markers need to be placed before any further activity occurs. This is especially <br />important given the irregular shape of the permit boundary. These markers should be surveyed in based on <br />the metes and bounds shown in Attachment 1. The Division requires the new boundary, shy and the <br />photographic documentation of the marker placement be submitted to the Division within 60 days. <br />2. If no mining activity is planned for more than 180 days, the Operator should apply for Temporary <br />Cessation. Attachment 2 includes a form. <br />3. The site is currently mostly reclaimed and the current $77,447 bond is adequate. However, before <br />additional material excavation occurs in the future, the bond should be re- evaluated by the Division. Please <br />notify the Division within 30 days after beginning additional excavation or other disturbance. <br />4. A noxious weed control plan should be implemented to prevent tamarisk from becoming a problem again. <br />5.
