c. Owner's names, types of structures and locations of all significant, valuable, and permanent
man -made structures within 200' of the affected land. See attached map and copy of an executed
structure agreement between the Authority and United Water and Sanitation District — the owner
of a slurry wall within 200' of the proposed concrete box culvert under Nome Street. I have also
sent notices to three other owners of adjacent structures by certified mail. Proof of mailing is
attached in Exhibit S.
EXHIBIT E — Reclamation Plan (Rule 6.4.5)
6. Reclamation Plan. See attached Reclamation Plan describing details of the proposed final
reclamation of the former gravel pit together with grading seeding information. As discussed, we
have re- designed the grading so that there is a minimum of 2' fill above the static water elevation.
7. E -470 Construction. A portion of the E -470 highway has been constructed in the north- eastern
corner of the permit area. This highway is the intended post- mining use for this part of the permit
area.
EXHIBIT F — Reclamation Plan May (Rule 6.4.6)
8. a. Expected physical appearance of the reclaimed land. See attached plan sheet 5 which shows
elevations and contours of the finished grades after the lake has been filled.
b. The final land use for the permit is pasture land with the exception of the north- eastern corner
which is now part of the E -470 highway.
The Authority is aware of our obligation to address the issue of evaporative water loss from the lake
surface per Rule 3.1.6. We have been working for many years to try to sell the property but have not been
successful in finding a buyer. We also investigated the possibility of preparing an augmentation plan but
we do not have sufficient water rights to make that financially viable. Accordingly, in 2013 our Board of
Directors decided that we should proceed to fill the lake. This solution will remove the open water surface
and therefore eliminate the evaporative loss issue.
Finally, as requested and in accordance with Rule 1.6.1(2), the municipalities within 2 miles of the
mining operation are listed on the attached Exhibit P.
Please call Neil Thomson at (303) 537 -3712 if you need more information or have questions about the
application. Alternatively, I'd be happy to come to your offices to meet and discuss the application.
Sincqly,
Neil G. omson P.E.
Director of Engineering and Roadway Maintenance
E -470 Public Highway Authority
