c. Owner's names, types of structures and locations of all significant, valuable, and permanent <br />man -made structures within 200' of the affected land. See attached map and copy of an executed <br />structure agreement between the Authority and United Water and Sanitation District — the owner <br />of a slurry wall within 200' of the proposed concrete box culvert under Nome Street. I have also <br />sent notices to three other owners of adjacent structures by certified mail. Proof of mailing is <br />attached in Exhibit S. <br />EXHIBIT E — Reclamation Plan (Rule 6.4.5) <br />6. Reclamation Plan. See attached Reclamation Plan describing details of the proposed final <br />reclamation of the former gravel pit together with grading seeding information. As discussed, we <br />have re- designed the grading so that there is a minimum of 2' fill above the static water elevation. <br />7. E -470 Construction. A portion of the E -470 highway has been constructed in the north- eastern <br />corner of the permit area. This highway is the intended post- mining use for this part of the permit <br />area. <br />EXHIBIT F — Reclamation Plan May (Rule 6.4.6) <br />8. a. Expected physical appearance of the reclaimed land. See attached plan sheet 5 which shows <br />elevations and contours of the finished grades after the lake has been filled. <br />b. The final land use for the permit is pasture land with the exception of the north- eastern corner <br />which is now part of the E -470 highway. <br />The Authority is aware of our obligation to address the issue of evaporative water loss from the lake <br />surface per Rule 3.1.6. We have been working for many years to try to sell the property but have not been <br />successful in finding a buyer. We also investigated the possibility of preparing an augmentation plan but <br />we do not have sufficient water rights to make that financially viable. Accordingly, in 2013 our Board of <br />Directors decided that we should proceed to fill the lake. This solution will remove the open water surface <br />and therefore eliminate the evaporative loss issue. <br />Finally, as requested and in accordance with Rule 1.6.1(2), the municipalities within 2 miles of the <br />mining operation are listed on the attached Exhibit P. <br />Please call Neil Thomson at (303) 537 -3712 if you need more information or have questions about the <br />application.
