Ron Roberts and Jim Smith <br />represented CC &V as the Construction Manager and Construction Superintendent, <br />respectively, during the course of the work. All quantities and testing frequencies presented <br />in this report are current as of August 2011, which represents completion of the 2011 Phase <br />5 VLF OSA 2nd Portion, as detailed in this report. <br />1.2 Construction Quality Assurance /Construction Quality Control <br />AMEC provided CQA and CQC monitoring and testing services, and prepared this report <br />describing the monitoring and test results for the 2011 Phase 5 VLF OSA 2nd Portion. <br />As -built drawings and supporting CQA monitoring and testing documentation are presented <br />in appendices attached to this report. <br />1.2.1 CQA Monitoring of Earthworks <br />AMEC provided CQC testing and CQA testing and monitoring of earthworks for the 2011 <br />Phase 5 VLF OSA 2nd Portion. CQA testing and monitoring of earthworks was provided for <br />in -place materials in accordance with the AMEC Earthworks CQA Plan. The earthworks <br />material specifications for this project are summarized in Table 1. CQC earthworks testing <br />was provided to support the contractor during the manufacture of materials and prior to <br />material placement. The CQC test results are not included in this report because these <br />tests were intended to assist the contractor in materials preparation rather than record <br />testing. <br />AMEC provided CQA laboratory and field testing, and monitored earthworks construction <br />activities as follows: <br />• AMEC monitored topsoil stripping, site grading, including slope <br />re- contouring and fill placement, lift thickness, moisture conditioning, <br />temperatures, compactive effort, in -place nuclear density testing, and use <br />of suitable material for Structural Fill (SF).
