Hipwell <br /> Geotechnical Consulting <br /> 3305 Calle Cuervo N/W #1218 <br /> Albuquerque NM 67114 <br /> RE: Well Permit No. 49219-F <br /> Dear Mr. Hipwell: <br /> Your request for an extension of time to complete the referenced well has been received and reviewed. <br /> You are hereby given notice that an extension of one year to November 21, 1999 by the State Engineer <br /> pursuant to Colorado Revised Statute 37-90-137(3)(a)(II). A copy of the amended well permit is enclosed. <br /> Once the well is completed a Notice of Completion must be filed by the well owner prior to the <br /> expiration date. Well completion and pump installation reports, including well and permanent pump test data, <br /> must be submitted to this office by the contractor. After water from the well has been put to beneficial use <br /> notice of Commencement of Beneficial Use must be submitted to this office within 30 days by the well owner. <br /> Forms are available from well contractors, pump installers, and Colorado Division of Water Resources offices. <br /> Pursuant to the Statewide Nontributary Ground Water Rules, if the name and license number of the <br /> licensed water well contractor or name of the private driller who will drill the well was not known at the time <br /> the permit application was submitted, or the listed licensed water well contractor or private driller who will <br /> drill the well is changed prior to construction, the permittee shall notify the State Engineer in writing of the <br /> name and license number of the new water well contractor or name of the private driller ten days prior to the <br /> commencement of construction of the well.
