A) Control Structures
No work performed during the week.
B) Best Management Practices
Best Management Practices are being observed throughout the Phase 5 VLF Project. Storm water
controls were maintained throughout the week. Precipitation not substantial throughout the week.
V) VLF Primary Underdrain
No work performed during the week.
VI)VLF Geosynthetics
No work performed during the week.
CQA ACTIVITIES:
SUMMARY:
The CQA activities performed consisted of observing, documenting and recording the construction
activities of the contractor.
AMEC observed and documented ambient and fill temperatures, as well as the placement of structural fill
within the Phase 5 VLF fill area.
AMEC observed clearing, grubbing, and stripping topsoil at the East Cresson Clay Borrow.
AMEC observed construction of the ADR/Crushing waterlines.
AMEC observed confirmatory drilling at Underground Workings #3009 and #3010 constantly.
AMEC observed the collapsing of the crown pillar at Underground Workings #3006 and #3007
constantly.
Testing Summary
Laboratory testing continued (Particle Size Distribution, Atterberg Limits, and Moisture - Density
Compaction testing) for sampled materials.

Structural fill placement within the Phase 5 VLF footprint will continue.
