Cripple Creek & Victor Gold Mining Company Cresson Project ameO Quality Assurance Monitoring and Test Results Load Out Bin Relocation July 7, 2009 1.0 INTRODUCTION Cripple Creek & Victor Gold Mining Company (CC &V) operates the Cresson Project, located in Teller County, Colorado, near the city of Victor. The Cresson Project is a mining and ore processing and beneficiation facility that is composed of surface mines as well as a valley leach facility (VLF); an adsorption, desorption, and recovery (ADR) plant; and associated infrastructure, all of which were designed and constructed to meet or exceed the requirements established by CRS §34 -32 -101 et seq. and regulations promulgated there under by the Mined Land Reclamation Board (MLRB). The work associated with the Cresson Project is being performed under specific criteria established in Amendment No. 8 of Permit Number M- 1980 -244 (Permit), as approved by the MLRB. This Report documents the Construction Quality Assurance (CQA) and Construction Quality Control (CQC) services provided by AMEC Earth and Environmental (AMEC) from March 2009 through May 2009 for the relocation of the VLF Load Out Bin (LOB). The general LOB Relocation site plan is presented on Figure 1, and the LOB pad certification boundary is presented on Figure 2. 1.1 Project Description The existing Cresson Project was constructed in phases from 1994 through 2004 (Phases I through IV). Phases I and II were permitted in 1994 and constructed during the 1994 through 1996 construction seasons. Phase III was permitted in 1999 and construction was initiated the same year, with completion in 2000. Phase IV was permitted in 2000 and construction was completed in 2004. Phase 5 was permitted in 2008 and is currently under construction. As part of the construction activities, the LOB is being moved from its current location to a new pad located to the north end of the VLF, as shown on Figure 2. 1.2 Construction Quality Assurance /Quality Control AMEC provided CQA and CQC monitoring and testing services, and prepared this report describing the monitoring and test results for the LOB relocation. As -built drawings and supporting CQA and CQC monitoring and testing documentation are presented in appendices attached to this report.
