Cripple Creek & Victor Gold Mining Company <br />Cresson Project ameO <br />Quality Assurance Monitoring and Test Results <br />Load Out Bin Relocation <br />July 7, 2009 <br />1.2.1 CQA Monitoring of Earthworks <br />AMEC provided CQC testing and CQA testing and monitoring of earthworks for the LOB <br />relocation. CQA testing and monitoring of earthworks was provided for in -place materials in <br />accordance with the AMEC Earthworks CQA Plan. The earthworks material specifications for <br />this project are summarized in Table 1. CQC earthworks testing was provided to support the <br />contractor during the manufacture of materials and prior to material placement. The CQC test <br />results are not included in this report because these tests were intended to assist Ames <br />(Earthworks Contractor) in materials preparation rather than record testing. <br />AMEC provided CQA laboratory and field testing, and monitored earthworks construction <br />activities as follows: <br />AMEC visually monitored and documented the remediation of underground workings <br />within the LOB footprint, including verification that Ames excavated underground <br />workings to the minimum specified dimensions and then installed Coarse Shaft Backfill <br />(CSB), Concrete Plug (CP), Cemented Rockfill (CR), Fine Shaft Backfill (FSB), Select <br />Structural Fill (SSF), Structural Fill (SF), and geogrid, as specified on the Issued For <br />Construction (IFC) drawings. <br />AMEC monitored site grading, including slope re- contouring and fill placement, lift <br />thickness, moisture conditioning, temperatures, compactive effort, and use of suitable <br />material for SF. AMEC performed CQA laboratory testing of grain size and Atterberg <br />limits. <br />AMEC visually monitored subgrade preparation to confirm that the subgrade surface <br />was unyielding, free of organic matter and other unsuitable materials, and that the <br />subgrade surface was compacted prior to Soil Liner Fill (SLF) placement. <br />AMEC visually monitored the placement and compaction of SLF including observation of <br />depth verification performed by Ames, particle size, moisture conditioning, and <br />compaction of the fill. AMEC performed in -place density and moisture content testing, <br />and performed CQA laboratory testing of grain size, moisture content, Atterberg limits, <br />compaction, and permeability. AMEC also provided visual monitoring of the SLF surface <br />preparation and moisture conditioning for geomembrane deployment, including <br />verification that the surface was proof - rolled, compacted, and hand - worked so as to be <br />free of desiccation cracks, abrupt grade changes, and sharp stones or objects. AMEC <br />also observed the backfill and compaction of the anchor trenches. <br />AMEC visually monitored the placement of Drain Cover Fill (DCF) within the LOB <br />footprint, including verification of lift thickness and grade control, identification and <br />SIPROJECTS\1125E CCV CQA \H3 - CONSTRUCTION \LOB CERT REPORT\REPORT BODY\LOB CERT REPORT 2009.07.01.DOC 2 <br />
