Cripple Creek & Victor Gold Mining Company <br />Cresson Project <br />Quality Assurance Monitoring and Test Results <br />Load Out Bin Relocation <br />July 7, 2009 <br />■ Caterpillar 330 Trackhoe (2); <br />■ Caterpillar 375 Trackhoe; <br />■ Caterpillar 385 Trackhoe; <br />■ John Deere 850 Rubber Tire Backhoe; <br />■ Komatsu 800 Trackhoe; <br />■ Volvo A40D Haul Trucks (8); <br />■ T360 Forklift <br />■ Caterpillar 777 Haul Trucks (6); and <br />■ Tamrock 700 and 1100 Drill Rigs (6 total). <br />ameO <br />Table 2 presents a summary of the weather during the period of April 3rd through May 6th, 2009 <br />for the LOB relocation construction activities. <br />2.1 Underground Workings Remediation <br />Historic underground workings within the footprint of the LOB were remediated following the <br />recommended remediation plans presented on Drawing No. A290 of the IFC Drawings. <br />The majority of underground workings (listed on Table 3) were removed during site grading <br />activities. Only one working required remediation. This working (No. 2019 located at N 54641.4 <br />and E 37585.9) consisted of a shallow shaft (approximate) 25 feet) with two small adits. <br />Exploratory drilling was conducted around the shaft to a minimum depth of 50 feet. The results <br />of the drilling determined that the adits did not extend beyond 5 feet from the shaft edge. This <br />working was remediated by removing the crown pillars of the adits and loose rock with a track <br />excavator. Once the loose rock was removed, the working was inspected by the Certifying <br />Engineer and then backfilled following the shallow shaft remediation presented on Drawing No. <br />A290. <br />All exploratory drillholes that did not intercept underground workings were completely grouted <br />with bentonite slurry prior to remediating the working. <br />2.2 Site Grading <br />Prior to SF placement, Ames conditioned and compacted the subgrade surface of the fill area. <br />veAMEC monitored the preparation of the subgrade and verified that it met project specifications. <br />S: \PROJECTS \1125E CCV CQA \H3 - CONSTRUCTION \LOB CERT REPORT\REPORT BODY\LOB CERT REPORT 2009.07.01.DOC 6 <br />
