Box 191 <br /> 100 North 3rd Street <br /> Victor, Colorado 80860 <br /> Attention: Mr. Ron Roberts <br /> RE: 1999 ADR EXPANSION CONSTRUCTION QUALITY ASSURANCE (CQA) <br /> MONITORING <br /> Dear Ron: <br /> The Cripple Creek & Victor Gold Mining Company (CC&V) requested geotechnical <br /> Construction Quality Assurance (CQA) services be performed by Golder Associates Inc. <br /> (Golder) on the 1999 ADR Expansion. Construction was performed by Industrial System <br /> Inc. (ISI), Ames Construction Inc. (Ames), and Poly-flex Construction Inc. (Poly-flex). <br /> During the period of August 8, 1999 through November 8, 1999, Golder observed and <br /> documented construction activities including, laboratory testing of soils and geomembrane, <br /> Backfill and Special Backfill placement, Soil Liner Fill (SLF) placement, geomembrane <br /> deployment, and Drain Cover Fill (DCF) placement. <br /> Golder performed laboratory tests for grainsize distribution, Atterberg limits, and <br /> compaction on samples of Backfill (SF-1), Special Backfill (SB-1 and SB-2), SLF (SLA-4), <br /> and DCF (DCF-1). Laboratory tests were conducted in accordance with applicable ASTM <br /> test methods and results met or exceeded the construction specifications. Laboratory test <br /> results and are presented in Attachment A. <br /> In memorandums to CC&V dated August 12, 1999 and September 25, 1999, Golder <br /> reviewed Special Backfill test results submitted by ISI and determined the material met the <br /> intent of the design. Therefore, Golder considered all of the materials acceptable. <br /> Field earthworks testing was conducted by Golder on Backfill and SLF placed during 1999 <br /> ADR Expansion construction. Golder performed in-place moisture density testing on <br /> Backfill material placed by ISI to verify that the material was placed in uniform layers of <br /> approximately 8 inches and compacted using a hand operated vibratory compactor to <br /> achieve a minimum relative density of 95% of modified Proctor dry density (ASTM-1557). <br /> SLF field testing of approximately 370 cubic yards of SLF included monitoring of <br /> placement and preparation for geomembrane deployment, testing in-place moisture and <br /> density, and monitoring depth.
