Box 191 <br /> 100 North Yd Street <br /> Victor, Colorado 80860 <br /> Attention: Mr. Ron Roberts <br /> RE: 1999 LOB AREA CONSTRUCTION QUALITY ASSURANCE (CQA) <br /> MONITORING <br /> Dear Ron: <br /> The Cripple Creek & Victor Gold Mining Company (CC&V) requested geotechnical <br /> Construction Quality Assurance (CQA) services to be performed by Golder Associates Inc. <br /> (Golder) on an area near the Load Out Bin (LOB). Construction activities were performed <br /> by Ames Construction Inc. (Ames) and Poly-flex Construction Inc. (Poly-flex) during the <br /> period of October 21, 1999 through October 25, 1999. Golder observed and documented <br /> construction activities including, subgrade preparation, anchor trench excavation, Soil <br /> Liner Fill (SLF) placement, geomembrane deployment, and Drain Cover Fill (DCF) <br /> placement. The testing frequency, material, construction sequence, etc. met or exceeded <br /> the required specifications for the project. <br /> Field earthworks testing was conducted by Golder on SLF placed in the LOB area. Soil <br /> Liner Fill field testing of approximately 40 cubic yards of SLF included monitoring of <br /> placement and preparation for geomembrane deployment, testing in-place moisture and <br /> density, and performing depth verification testing. Soil Liner Fill placement was <br /> performed in accordance with the 1999 Phase III VLF construction specifications. Results <br /> and approximate locations of field density testing performed on SLF are presented in <br /> Attachment A. Although no permeability testing was specifically performed on SLF placed <br /> at the LOB area, Golder has included a representative permeability test result of SLF from <br /> the 1999 Phase III VLF construction in Attachment A. <br /> To construct the LOB area in accordance with the 1999 Phase III specifications, Ames was <br /> required to excavate approximately 35 feet of anchor trench along the east side of the <br /> extension. In the location of the required anchor trench, bedrock was encountered at <br /> ground elevation, making the excavation of the Phase III design anchor trench (3' deep and <br /> 2' wide) difficult. To assist constructability, a 4 ft deep by 1 ft wide anchor trench was <br /> proposed as an alternative to the Phase III design anchor trench.
