Cripple Creek & Victor Gold Mining Company <br />PSES Pipe Rack Liner Extension <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />February 4, 2014 <br />amecI9 <br />Prior to the 80 -mil double -sided textured LLDPE geomembrane arrival to site, AMEC <br />performed one conformance test at a frequency of one test per 106,260 square feet. <br />The minimum specified testing frequency of one test per 150,000 square feet was <br />exceeded. Testing frequency was based on AMEC's inventory of the 80 -mil double - <br />sided textured LLDPE geomembrane. Conformance testing for the geomembrane <br />used to construct the PSES Pipe Rack Liner Extension met specifications. <br />Conformance test results are presented in Appendix K. <br />6.2 Deployment Monitoring <br />Geomembrane panel deployment monitoring performed by AMEC included: <br />• Observation of panel surface condition and panel overlap <br />• Assignment of panel numbers <br />• Verification of factory roll number and receipt of complete certification prior to <br />allowing deployment <br />• Measurement of sheet thickness <br />• Documentation of deployed panel lengths <br />• Development of panel layout record drawing <br />Prior to the deployment of GCL, AMEC and ECA verified that the random fill subgrade <br />was free of soft spots, protrusions, angular particles, abrupt grade changes, and loose <br />soil. AMEC and ECA also verified the subgrade surface was free of desiccation <br />cracks. Where the surface of the subgrade did not meet specifications, Ames repaired <br />the area to meet project specifications. ECA provided written certification of subgrade <br />acceptance prior to the placement of GCL. The subgrade acceptance certificates are <br />presented in Appendix G. <br />GCL was placed over the approved random fill subgrade by ECA. AMEC observed <br />the placement of the GCL to ensure that appropriate overlap was achieved and that <br />the GCL was placed in accordance with the Technical Specifications. <br />AMEC assigned each geomembrane panel a number for this project, they were <br />designated as P1 through P13. <br />AMEC marked the panel designation and roll number directly on each panel after it <br />was deployed. The geomembrane record drawing, showing the PSES Pipe Rack <br />Liner Extension's geomembrane panel layout and destructive testing is presented as <br />Sheet 2 of 2 in Appendix A. <br />During geomembrane deployment, AMEC measured sheet thickness with digital <br />calipers. The sheet thickness was measured at five random locations along each <br />edge of each geomembrane panel. The average thickness specified for the 80 -mil <br />geomembrane was 80 mils.
