Cripple Creek & Victor Gold Mining Company <br />PSES Pipe Rack Liner Extension <br />Final Report ameO <br />Quality Assurance Monitoring &Test Results <br />February 4, 2014 <br />6.0 GEOMEMBRANE CQA TESTING AND MONITORING <br />The following sections describe the CQA activities performed by AMEC for the <br />geomembrane installed in the PSES Pipe Rack Liner Extension. <br />In accordance with the AMEC Geosynthetics CQA Plan (Section 1400.2 of the <br />Specifications), when an AMEC monitor observed deficiencies, the monitor determined <br />the nature and extent of the problem and notified ECA and /or CC &V. When <br />unsatisfactory test results identified a deficiency, AMEC performed additional tests to <br />define the extent of the deficient area. ECA corrected deficiencies to meet the <br />requirements of the specifications, and AMEC re- tested the corrected deficiency prior <br />to any additional related work. <br />CQA activities conducted by AMEC during the installation of the LLDPE geomembrane <br />at the PSES Pipe Rack Liner Extension consisted of the following: <br />• Conformance sampling and testing of the geomembrane <br />• Observation and approval of the random fill subgrade prior to GCL placement <br />• Observation of GCL placement to ensure adequate overlap and that placement <br />was performed in accordance with the Technical Specifications prior to <br />geomembrane placement <br />• Observation and documentation of geomembrane deployment, including verifying <br />sheet thickness and overlap, and documenting panel layout <br />• Observation of trial seam sample testing and documentation of results <br />• Observation and documentation of field seaming <br />• Identification of destructive seam sample test locations, observation of destructive <br />seam sample testing, and documentation of test results <br />• Observation and documentation of geomembrane repairs <br />• Observation and documentation of non - destructive pressure testing of fusion <br />seams and vacuum testing of extrusion seams and repairs <br />The following sections present a detailed description of each of the above activities. <br />6.1 Geomembrane Conformance Testing <br />Geomembrane manufactured by Agru America was installed for the PSES Pipe Rack <br />Liner Expansion. A TRI Environmental (TRI) (AMEC subcontractor) representative <br />performed geomembrane conformance sampling at the Agru plant in Georgetown, <br />South Carolina. Samples were sent to TRI's geosynthetic laboratory for conformance <br />testing. Geomembrane rolls were not used on the project unless approved by AMEC <br />based on conformance test results. <br />12 <br />
