Cripple Creek & Victor Gold Mining Company <br />Mill Site Earthworks Project <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 31, 2014 <br />ameO <br />a frequency of one test per 133,343 square feet. Nine conformance tests were <br />performed on the double -sided textured geomembrane, at a frequency of one test per <br />32,501 square feet. The minimum specified testing frequency of one test per 150,000 <br />square feet was exceeded. Conformance testing for the 80 -mil single -sided and <br />double -sided textured LLDPE geomembrane used to construct the Projects liner <br />system met Project Specifications. Conformance test results are presented in <br />Appendix K. <br />6.2 Geogrid Conformance Testing <br />Quality Control certifications provided for the geogrid is presented in Appendix M.4. <br />Conformance testing results performed on geogrid used in the remediation of the <br />underground workings within the footprint of the Mill Site Earthworks Project are <br />presented in Appendix M.S. AMEC monitored geogrid installation. <br />Conformance testing for geogrid used for Mill Site Earthworks Project construction met <br />Project Specifications. <br />6.3 Deployment Monitoring <br />Geomembrane panel deployment monitoring performed by AMEC included: <br />• Observation of panel surface condition and panel overlap <br />• Assignment of panel numbers <br />• Verification of factory roll number and receipt of complete certification prior to <br />allowing deployment <br />• Measurement of sheet thickness <br />• Documentation of deployed panel lengths <br />• Development of panel layout record drawing <br />Prior to the deployment of geomembrane, AMEC and Lone Star verified that the <br />subgrade was free of soft spots, protrusions, angular particles, abrupt grade changes, <br />and loose soil. AMEC and Lone Star verified the SLF surface had been proof rolled <br />and was free of desiccation cracks. Where the surface of the SLF did not meet Project <br />Specifications, Ames repaired the area and re- compacted the surface. AMEC <br />prepared SLF acceptance forms, after inspections of the finished surface; a Lone Star <br />and AMEC representative signed the acceptance forms. The acceptance certificates <br />are presented in Appendix G. <br />AMEC assigned each geomembrane panel a number. Single -sided and double -sided <br />textured 80 -mil LLDPE geomembrane panels deployed were designated as P4 -P1 <br />through P4 -P6 and P1 through P498. Panel numbers beginning with "P4 -" designate <br />23 <br />
