Cripple Creek & Victor Gold Mining Company <br />Mill Site Earthworks Project <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 31, 2014 <br />6.0 GEOMEMBRANE CQA TESTING AND MONITORING <br />mlle <br />The following sections describe the CQA activities performed by AMEC for the <br />geomembrane installed for the Project. <br />In accordance with the AMEC Geosynthetics CQA Plan (Section 01400.2 of the <br />Project Specifications in Appendix E), when an AMEC monitor observed deficiencies, <br />the monitor determined the nature and extent of the problem by performing additional <br />testing to define the extent of the deficient area. Lone Star and /or CC &V were <br />notified of the deficiency for repairs. Lone Star corrected deficiencies to meet the <br />requirements of the Project Specifications, and AMEC re- tested the corrected <br />deficiency prior to any additional related work. <br />CQA activities conducted by AMEC during the installation of the LLDPE geomembrane <br />for the Project consisted of the following: <br />• Conformance sampling and testing of the geomembrane <br />• Observation and approval of the SLF surface prior to deployment of 80 -mil single - <br />sided and double -sided textured LLDPE geomembrane <br />• Observation and documentation of geomembrane deployment, including verifying <br />sheet thickness and overlap, and documenting panel layout <br />• Observation of trial seam sample testing and documentation of results <br />• Observation and documentation of field seaming <br />• Identification of destructive seam sample test locations, observation of destructive <br />seam sample testing, and documentation of test results <br />• Observation and documentation of geomembrane repairs <br />• Observation and documentation of non - destructive pressure testing of fusion <br />seams and vacuum testing of extrusion seams and repairs <br />The following sections present a detailed description of each of the above activities. <br />6.1 Geomembrane Conformance Testing <br />Geomembrane manufactured by Poly -Flex, Inc. and GSE Environmental was installed <br />for this Project. A TRI /Environmental (TRI) (AMEC subcontractor) representative <br />performed geomembrane conformance sampling at the Poly -Flex, Inc. plant in Grand <br />Prairie, Texas and GSE plant in Houston, Texas. Samples were sent to TRI's <br />geosynthetic laboratory for conformance testing. Geomembrane rolls were not used <br />on the project unless approved by AMEC based on conformance test results. <br />Prior to the 80 -mil single -sided and double -sided textured geomembrane arrival to site, <br />AMEC performed 11 conformance tests on the single -sided textured geomembrane, at <br />22 <br />
