PERMIT #: M -1979 -045 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: January 30, 2014 <br />The site is permitted for 75.74 acres. The 2013 annual report submitted by the Operator indicated 67.50 acres <br />of disturbance and 3.55 acres of reclamation at the site. The site was active during the inspection. The <br />disturbance at the site was accurately depicted in the annual report and map. <br />The site is mined for clay and sand and gravel. Lakewood Brick operates the clay mine, located in the north <br />central part of the permit area. Bestway Concrete Company operates the sand and gravel operation in the <br />south and southeast part of the Church Pit and the adjacent Rocky Flats Pit. An agreement between Bestway <br />and Lakewood Brick exists for material mined from both operations to be processed at the Church Pit. <br />Standing water and snow were observed in the clay pit and a process water pond was observed near the sand <br />and gravel processing plant. The Operator did not respond to the problem cited in the last inspection report <br />for exposed ground water at the site. The problem is reissued and will require a response from the Operator <br />as stated on the first page of this inspection report. <br />The entrance sign and boundary markers were observed during the inspection as required by Rule 3.1.12. The <br />boundary markers were GPS surveyed by the Division with a Trimble Juno 3B. The survey indicated a permit <br />area of 75.03 acres, which appears accurate due to the southern permit markers being offset to be in line with <br />the fence line. A map indicating the results of the survey is attached. The Division holds a financial warranty <br />in the amount of $319,000.00, which appears adequate to reclaim the site based on this inspection. <br />Photographs taken during the inspection are attached below. <br />Page 2 of 4 <br />
