Cripple Creek & Victor Gold Mining Company <br />Phase 5 Slide Repair <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 14, 2014 <br />ameO <br />the #200 sieve by less than 2 percent when sampled, it was brought to Ames attention <br />and immediately and corrected as can be seen in sample Phase V -2. <br />4.1.2 Moisture Content <br />AMEC measured oven moisture contents of DCF as part of the CQA program. <br />Moisture content data are summarized in Table 4. <br />4.1.3 Atterberg Limits <br />AMEC performed two CQA Atterberg limits tests for DCF. The testing frequency <br />(approximately one test per 2,680 cubic yards) exceeded the minimum specified <br />testing frequency of one test per 20,000 cubic yards. The project specifications called <br />for the material to be non - plastic and both samples yielded non - plastic results. The <br />DCF CQA Atterberg limits met project specifications, and results are presented in <br />Appendix H and summarized in Table 4. <br />4.2 Field Testing and Monitoring <br />AMEC observed earthwork fill operations at the Phase 5 Slide Repair to verify, when <br />necessary, lift thickness and acceptable moisture conditions. <br />4.2.1 Soil Liner Fill Preparation <br />AMEC monitored SLF (subgrade) preparation for compliance with the Technical <br />Specifications. It was verified that Ames removed protruding rocks from the surface <br />prior to geomembrane placement; and the surface was moisture conditioned, smooth, <br />and unyielding prior to geomembrane placement. <br />4.2.2 Drain Cover Fill <br />AMEC provided visual monitoring of all DCF placement. One AMEC monitor and one <br />Ames labourer per dozer monitored the placement of DCF, unless two dozers were <br />within approximately 50 feet of each other during DCF placement. Where present, <br />AMEC verified that wrinkles in the geomembrane did not fold over. PVC grade <br />markers were placed on a maximum 50 foot by 50 foot grid and at grade breaks; and <br />all fill materials were suitable. AMEC continuously inspected geomembrane during <br />DCF placement; marked areas for repair if required; and verified that all repairs were <br />completed to project specifications.
