Cripple Creek & Victor Gold Mining Company
Phase 5 Slide Repair
Final Report
Quality Assurance Monitoring & Test Results
January 14, 2014
ameO
the #200 sieve by less than 2 percent when sampled, it was brought to Ames attention
and immediately and corrected as can be seen in sample Phase V -2.
4.1.2 Moisture Content
AMEC measured oven moisture contents of DCF as part of the CQA program.
Moisture content data are summarized in Table 4.
4.1.3 Atterberg Limits
AMEC performed two CQA Atterberg limits tests for DCF. The testing frequency
(approximately one test per 2,680 cubic yards) exceeded the minimum specified
testing frequency of one test per 20,000 cubic yards. The project specifications called
for the material to be non - plastic and both samples yielded non - plastic results. The
DCF CQA Atterberg limits met project specifications, and results are presented in
Appendix H and summarized in Table 4.
4.2 Field Testing and Monitoring
AMEC observed earthwork fill operations at the Phase 5 Slide Repair to verify, when
necessary, lift thickness and acceptable moisture conditions.
4.2.1 Soil Liner Fill Preparation
AMEC monitored SLF (subgrade) preparation for compliance with the Technical
Specifications. It was verified that Ames removed protruding rocks from the surface
prior to geomembrane placement; and the surface was moisture conditioned, smooth,
and unyielding prior to geomembrane placement.
4.2.2 Drain Cover Fill
AMEC provided visual monitoring of all DCF placement. One AMEC monitor and one
Ames labourer per dozer monitored the placement of DCF, unless two dozers were
within approximately 50 feet of each other during DCF placement. Where present,
AMEC verified that wrinkles in the geomembrane did not fold over. PVC grade
markers were placed on a maximum 50 foot by 50 foot grid and at grade breaks; and
all fill materials were suitable. AMEC continuously inspected geomembrane during
DCF placement; marked areas for repair if required; and verified that all repairs were
completed to project specifications. After final grading, AMEC monitored removal of
PVC grade markers.
s1projects \1568 cc &v -ph v slide repair cqa \11.0 certification report \cc &v roc report. docx 8
