Cripple Creek & Victor Gold Mining Company <br />Laboratory Liner Extension <br />Final Report ameO <br />Quality Assurance Monitoring &Test Results <br />January 14, 2014 <br />2.0 SUMMARY OF EARTHWORK CONSTRUCTION <br />Earthworks activities associated with the Laboratory Liner Extension included the <br />following: <br />• Site excavation and re- shaping per the drawings <br />• Preparation and acceptance of the subgrade prior to GCL deployment <br />• Placement of rebar for the Laboratory Liner Extension concrete slab <br />• Installation of all concrete embeds and accessories required by the plans and <br />specifications <br />• Installation of a 6 -inch concrete slab with curbs and tapered sections as per plan <br />Weather data for the Laboratory Liner Extension construction activities from the period <br />of January 2013 are presented in Table 1. <br />2.1 CQA Monitoring of Earthworks <br />AMEC provided CQA testing and monitoring of earthworks for the Laboratory Liner <br />Extension. CQA monitoring of earthworks was provided for placed materials in <br />accordance with the AMEC Earthworks CQA Plan. AMEC monitored earthworks <br />construction activities as follows: <br />• AMEC monitored site excavation and grading, including slope re- contouring, fill <br />placement, lift thickness, moisture conditioning, temperatures, and compactive <br />efforts <br />• AMEC visually monitored subgrade preparation to confirm that the subgrade was <br />unyielding; free of organic matter and other unsuitable materials; and the subgrade <br />was compacted using a smooth drum vibratory compactor prior to GCL deployment <br />• AMEC observed the backfill around the newly placed slab and at the connection to <br />the existing liner to verify compliance with specifications and ensure no damage <br />was done to the liner during the backfilling process <br />• AMEC monitored ambient air temperatures and the temperatures of all fills, in <br />accordance with project specifications <br />• AMEC monitored the placement of rebar to ensure all bars were placed in <br />accordance with the drawings and all materials used for reinforcing met <br />specifications <br />• AMEC monitored the placement of embeds and all associated hardware and <br />accessories to ensure they were meeting the manufacturers installation <br />requirements <br />sAprojects \1125m -cc &v lab liner exlension\h2 - design \lab liner extension \roc report\lab liner roc report.docx 3 <br />
