Compliance With Mine Plan Y - Support Facilities Not On-site <br /> NA -Other NA - Special Categories Of Mining <br /> NA -Processing Waste Y -Topsoil <br /> COMMENTS <br /> This was a complete inspection and an aerial inspection of the New Horizon North Mine conducted by Mike <br /> Boulay of the Division of Reclamation,Mining and Safety Grand Junction Field Office. A ground inspection of <br /> the site was conducted on December 17,2013. Ross Gubka of Western Fuels-Colorado(WFC)was present during <br /> the entire ground inspection. Weather conditions were cold and clear and ground conditions were partially snow <br /> covered. <br /> Aerial photographs of the site were taken by Harry Ranney of the DRMS at approximately 13:45 on the following <br /> day(December 18,2013)after the ground inspection was conducted. Four photographs were taken during the <br /> flyover. An aerial overview of the New Horizon North Mine is provided in the attached Photograph No. 1. <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(l): <br /> All required records were on site and available for review. Please see the attached records checklist at the end of <br /> this inspection report. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage —Acid and Toxic Materials 4.05.8; Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> Sedimentation Pond NHN-001 was inspected. The pond is containing water and is frozen at the surface <br /> (Photograph No. 2). The water level is well below the level of the primary spillway. The pond appears to be well <br /> constructed with no problems noted. Pond inlets,embankments and discharge structures were stable.
