Compliance With Mine Plan NA - Support Facilities Not On-site <br /> NA -Other NA - Special Categories Of Mining <br /> NA -Processing Waste Y -Topsoil <br /> COMMENTS <br /> This was a complete inspection and aerial inspection of the New Horizon Mine conducted by Mike Boulay of the <br /> Division of Reclamation,Mining and Safety Grand Junction Field Office. A ground inspection of the site was <br /> conducted on December 17,2013. Ross Gubka of Western Fuels-Colorado (WFC)was present during the entire <br /> ground inspection. Weather conditions were cold and clear and ground conditions were mostly snow covered. <br /> There was no activity at the New Horizon Mine during this inspection. <br /> Aerial photographs of the site were taken by Harry Ranney of the DRMS at 13:45 on the following day(December <br /> 18,2013). <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(1): <br /> All required records were on site and available for review. Please see the attached records checklist at the end of <br /> this inspection report. <br /> BACKFILL and GRADING—Rule 4.14 <br /> Contemporaneous Reclamation 4.14.1;Approximate Original Contour 4.14.2;Highwall Elimination 4.14.1(2)(f); <br /> Steep Slopes 4.14.2,4.27;Handling of Acid and Toxic Materials 4.14.3; Stabilization of Rills and Gullies 4.14.6: <br /> Much of the site has been backfilled and graded. The final pit to be backfilled is shown in the attached aerial <br /> photograph(Photo No. 1).
