Maintenance items identified during inspection: <br />• Please address the constructed berm in the southern segment of Diversion 5 -D2. The land <br />to the west appears to have been used as a borrow area to construct a land bridge across <br />the diversion channel, precluding flow through this diversion. This may be remedied by <br />clearing the berm from the channel, rerouting the flow around the berm, or placing a <br />culvert to allow flow directly through the channel. Placing a culvert would require a <br />Technical Revision application. <br />• Minor tree debris is beginning to accumulate in Diversion 5 -D2 and should be monitored <br />or removed before it becomes a problem. <br />• Please clean out the following culverts: 28 (north side is % full), 34, 48, 63 (east side is <br />blocked), the blocked discharge culvert from Pond 5 -P3. <br />• Rocks are accumulating in the #5D (Mine 5 dewatering, outfall 003) discharge flume. <br />Although water is no longer being pumped from Mine 5, the outlet should be maintained <br />to allow flow as designed. <br />• Animal burrows were noted in several pond embankments. This is a standard problem at <br />this site and should be monitored / repaired as usual. <br />• Please update mine ID signs at entrances to the site (Rule 4.02.2). The DRMS permit <br />number is printed as "C- 081 -044" rather than the most appropriate "C- 1981 -044" or <br />acceptable "C -81- 044." High quality stickers may be used to update the permit number <br />rather than replacing the signs. <br />• Two markers on topsoil stockpiles near the waste disposal site have fallen and should be <br />replaced. <br />• Update records to include missing files (underground mining landowner notifications, <br />documents pertaining to the SL -1 bond release). <br />• Quarterly pond inspections are conducted by a P.E.; please ensure inspection reports are <br />certified by a P.E. at least once annually if being used as the annual certification in <br />accordance with Rule 4.05.9(14)(a). <br />No compliance issues were noted and no enforcement actions were taken as a result of this <br />inspection. <br />in <br />
