PERMIT #: M- 1986 -015 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: November 15, 2013 <br />(dewatering, Phase 2 backfill and water impoundment, and slurry wall construction), and the collapsed soils; it <br />is not enough to sufficiently prove or disprove the mine is directly responsible for initiating the observed <br />collapsed soils. Other factors such as the recent drought, infrequent flooding and changes in land use may also <br />be contributing factors. <br />Summary: <br />1. The DRMS strongly encourages the mine and the property owners to communicate and cooperate with <br />each other to mitigate the problem to ensure public safety and conserve property values. <br />2. The Phase 2 stockpile erosion problem requires both a short term and long term solution: <br />A. Short term — The operator shall implement an Erosion and Fugitive Dust Control Plan on all the <br />stockpiles. If no Erosion and Fugitive Dust Control Plan has been provided, the operator shall <br />submit an Erosion and Fugitive Dust Control Plan for approval as a technical revision to the <br />permit along with the appropriate $216 revision fee to the Division by the corrective action date. <br />B. Long term — The operator shall notify the DRMS it has begun the removal /relocation of the <br />phase 2 stockpile on or before 2/28/2014 and provide photographic documentation by June 7, <br />2014 that the process is complete <br />3. The operator shall submit a Technical Revision, with the required $216 revision fee, to include an <br />updated Mining Plan Map showing all of the groundwater monitoring locations, all past groundwater <br />monitoring data, and well mitigation efforts by the operator for surrounding properties by the corrective <br />action due date. <br />4. If a Technical Revision for an Erosion and Fugitive Dust Control Plan is required (i.e., one does not <br />exist for this site), the new plan can be included in the Technical Revision for the required updated <br />groundwater monitoring plan and map. As the due dates are different for the two corrective actions, <br />please contact Tim Cazier at the DRMS and coordinate a schedule to implement both in an acceptable <br />time frame. <br />5. The operator committed to finishing the backfill in the Northwest corner of the northeast part of Phase 2. <br />References: <br />CGS. 2008.
