Liberty Mining, LLC <br />DRMS 110 -d Permit Application <br />October 2013 <br />• Concrete stormwater inlet structures will be placed on the up- gradient side of the <br />access road. The inlet will be placed at the appropriate grade to collect stormwater <br />from designated drainage basins as illustrated in the Drainage Improvement Plan. <br />• Culverts (HDPE) will be placed beneath the access road and grouted into the <br />stormwater inlet structures. Culvert discharge plunge pools will be constructed in <br />accordance of the Drainage Improvement Plan. <br />• Wattles will be installed on the down - gradient side of the access road and the mine <br />operations pad. The wattles will be installed as detailed in the Drainage <br />Improvement Plan. <br />Milestone 2: Construction tasks under this milestone are to be complete prior to the placement <br />of any new waste rock on the mine operations pad. <br />• Construction and associated surface grading of the catchment and sedimentation <br />ponds 1, 2, 3 and 4 will be completed to prevent stormwater run -on to the pad and to <br />prevent offsite stormwater runoff from the waste rock stockpile. <br />• Topsoil will be collected from the waste rock stockpile area in sections to allow for <br />placement of the waste rock. The topsoil will be stockpiled on the existing topsoil <br />stockpile and seeded for stabilization. <br />Milestone 3: Construction tasks under this milestone will be completed prior to the placement <br />of any ore on the surface of the mining operations pad. <br />• The ore stockpile pad concrete structure and ore pad sump pond are to be <br />constructed and pass final inspection prior to the placement of ore materials. <br />Milestone 4: The mine is to be reclaimed as described in the Mine Reclamation Plan <br />U (1 6) Quality Assurance and Quality Control Program <br />The Quality Assurance and Quality Control Program for the construction of stormwater diversion <br />structures, detention ponds, berms, check dams and the ore pad will consist of the engineered <br />design, construction observation, and routine field inspections after construction is complete. <br />Each of the proposed stormwater structures are designed and stamped by a licensed engineer. <br />The structures are designed to withstand a 100 year, 24 hour storm event. The ore pad is <br />designed to withstand the routine operations of stockpiling and loading material on the pad. <br />Prior to the construction of ston-nwater diversion structures and the ore pad, a representative <br />soil sample will be collected and submitted to a geotechnical lab for a Standard Proctor test to <br />detemtine proper compaction and moisture specifications for on -site materials. In the event that <br />additional materials are imported from an off -site location, a representative sample will be <br />collected from those materials and submitted for testing. <br />During construction, a qualified construction observer will be on site to document work <br />performed and correct any actions that may jeopardize the integrity of the structures. <br />Liberty Mining; LLC Moab, Utah SMA Ref. 5821897 Page 67 <br />
