Application Form, 8 pages (Original and copy) <br />2. Exhibits, 62 pages, including maps (two copies) <br />3. Proof of Posting Notice on site, 2 pages including pictures <br />4. Structure and Right to Enter Agreements for both landowners, 6 pages <br />5. Check for application fee of $2696. <br />The proofs of delivery of the applications to the Board of Montezuma County Commissioners and <br />the local Conservation District, and the placement of a public review copy in the Montezuma <br />County Clerk and Recorders Office (Exhibits Q and R) are located on the sixty -first page of the <br />package (Exhibit page 53). These were done today; this package is being mailed following <br />assembly and printing of the package. <br />This conversion is implemented, pursuant to an agreement between the Operator, the two <br />landowers, and Montezuma County, which will be preparing and submitting an Amendment <br />Application for M 1990 -025 (Hay Camp Pit). An application for Succession of Operator from Four <br />States Aggregates LLC to Montezuma County was submitted by hand, with supporting <br />documents and application, to the Division's Durango Office on 10 September 2013. <br />This conversion is the fourth of a series of actions for which applications are being submitted, as <br />discussed with both Ms. Pickford and Mr. Erickson of the Division's Durango Office: <br />a. Succession of Operator for M 1983 -028, Wallace Pit, from Oldcastle (Four Corners Materials) to <br />Stone Sand & Gravel, LLC. (done) <br />
