<br />(1) Financial Status Report (Attachment 1). - <br /> <br />(a) Each Federal agency shall require recipients to use the <br />standardized Financial Status Report to report the status of <br />funds. <br /> <br />(b) The Federal agency shall prescribe whether the report <br />shall be on a cash or accrual basis. If the Vederal agency <br />requires accrual information and the recipient's accounting <br />records are not normally kept on the accrual basis, the recipient <br />shall not be required to convert its accounting system, but shall <br />develop such accrual information through an analysis of the <br />documentation on hand. <br /> <br />(C'J 1[.(: r(:o(:r",'\ "'9(:['t.)' ~h"'-I"\ oE:tE:rm'lflE: thE: frequE:nc)' of thE: <br />Financial Status Report for each project or program considering <br />the size and complexity of the particular project or program. <br />However, the report shall not be required more frequently than <br />quarterly or less frequently than annually except as provided <br />above. A f'lnal report shall be required at the completion of the <br />cooperative agreement. <br /> <br />(d) Federal agencies shall require recipients to submit the <br />Financial Status Report (original and no more than two copies) no <br />later than 30 days after the end of each specified reporting <br />periOd for quarterly and semiannual reports, and 90 days for <br />annual and final reports. Extensions to reporting due dates may <br />be granted upon request of the recipient. <br /> <br />c. Except as noted below, only the following forms will te authorized <br />for recipients requesting reinbursements. <br /> <br />(1) Request for Advance or Reimbursement (Attachment 2). - <br /> <br />(al Each Federal agency shall adopt the Request for Advance <br />or Reimbursement as a standardized form. <br /> <br />(b) Recipients shall be authorized to submit requests for <br />reimbursements at least monthly. Federal agencies shall not <br />require the submission of more than the original and two copies of <br />the Request for Advance or Reimbursement. <br /> <br />d. When the Federal agencies need additional information in using <br />these forms or more frequent reports, the following shall be observed: <br /> <br />(1) When additional information is needed to comply with legislative <br />requirements, Federal agencies shall issue instructions to require <br />recipients to submit such information under the "Remarks" section of <br />the reports. <br /> <br />(2) When a Federal agency has determined that a recipients accounting <br />system does not meet the standards for financial management contained <br />in General Provision No.5, additional pertinent information to <br /> <br />5 <br />
