Reservoir Superintendent during an emergency situation or unusual <br /> <br />occurrence in immediate actions to take, who to notify, and what deci- <br /> <br />sions to make in event of loss of communications. An unusual occurrence <br /> <br />is defined as an event which takes place or a condition which develops <br /> <br />which is NOT NORMALLY ENCOUNTERED in the ROUTINE OPERATION of the dam <br /> <br />and reservoir and which MAY ENDANGER THE DAM or NECESSITATES either a <br /> <br />temporary or permanent REVISION of the OPERATING PROCEDURES. <br /> <br />The Dam Tender shall IMMEDIATELY REPORT the occurrence of unusual <br /> <br />conditions to the Chief, Chama Field Division, and the Projects <br /> <br />Superintendent by radio. The Projects Superintendent will notify <br /> <br />the Water Management Branch, regional office, by telephone. Depending <br /> <br />upon the nature of the development and the apparent urgency, the' <br /> <br />regional office shall notify the E&R Center and/or the Commissioner's <br /> <br />office. <br /> <br />All reporting officials shall maintain a diary (tape recordings, if <br /> <br />possible) on the preliminary form "Record of Telephone Report of Emer- <br /> <br />gency or Unusual Conditions and Facility Failures." <br /> <br />In general, initial reports should briefly include the following <br /> <br />information and should be documented on the "Record of Telephone Report." <br /> <br />A-S <br /> <br />- <br />
