Applications for rights-of-way should be filed with the District Manager <br />having Jurisdiction over the affected public lands. <br /> <br />2. Applications for rights-of-way must be filed on a form approved by the <br />Director, i.e., Standard Form 299. <br /> <br />3. Application content - how to complete Standard Form 299. <br /> <br />Item #1: The applicant should provide his/her name and address. The <br />mation given should be exactly as it is to appear in the right-of-way <br />Include the appropriate zip code. <br /> <br />Item #2: This item will be completed by individuals only if the applicant <br />has designated another party to act in his/her behalf with regard to the <br />right-of-way. It will also be completed by corporate and partnership appli- <br />cants to' provide BLM with the name and address of the person authorized to <br />act on behalf of the corporation or partnership on all matters relating to <br />the right-of-way. <br /> <br />infor- <br />grant. <br />, <br /> <br />Item #3: The applicant should provide a telephone number where he/she can be <br />reached during normal business hours. The same information should be provided <br />for any authorized agent. Be sure to include area codes. <br /> <br />Item #4: The information requested is self-explanatory. <br /> <br />If the applicant is a partnership, it must provide the name and <br />address of each partner. This information can be listed on a separate sheet <br />and attached to th~ application. <br /> <br />For corporate and partnershi p app 1 i cants, if nlore than one person has <br />been designated to act on behalf of the corporation or partnership with rega..d <br />to the right-of-way, the applicant must provide a list of those persons so <br />authorized and the information required under Item 12 above. This can also be <br />attached as a separate sheet. <br /> <br />A business entity is required under the regulations to certify that it is <br />authorized to conduct business in the State of Colorado. The applicant's sign- <br />ature on the application form is taken as such certification, so the business <br />entity applicant should aSSure itself that the person Signing the application <br />is authorized to~o act on behalf of the entity. <br /> <br />Item #5: The applicant should specify whether he/she is applying for a new <br />right-of-way; to amend an existing right-of-way; to renew an existing right- <br />Of-way; or to assign an existing right-of-way. If the application afrects <br />an existing right-of-way, the applicant should provide the serial number for <br />the existing authorization. <br />
