<br />Requests for determinations concerning single <br />structures on single lots and parcels of land <br />should be sent to the appropriate FEMA <br />Regional Office for processing. The Regional <br />Offices and their addresses are listed in <br />Appendix D. <br /> <br />Required Supporting Data <br /> <br />The requestor must provide data that are <br />sufficient to allow FEMA to verify the location of <br />the structure or parcel of land and that the <br />structure or parcel of land is not subject to <br />inundation during the base flood. For <br />determinations for structures, the following <br />data must be submitted: <br /> <br />. A copy of the recorded deed with the <br />legal descri ption of the property and the <br />deed book volume and page number, <br />beari ng the seal of the Recorder of <br />Deeds <br /> <br />. A copy of the recorded plat showi ng <br />both the location of the property and <br />the plat book volume and page number, <br />bearing the seal of the Recorder of <br />Deeds. If the pToperty is not recoTded on <br />a plat map, the requestor must provide <br />copies of a tax map or other suitable <br />maps to help FEMA accurately locate the <br />property. <br /> <br />. A topographic map, certified as-built by <br />a registered professional engineer or <br />licensed land surveyor, showing <br />structure locations (if applicable), <br />ground elevations, and the elevations of <br />the lowest finished grades adjacent to <br />the structures (if applicable). If the <br />request concerns an undeveloped parcel <br />of land, the submitted data must include <br />the elevation of the lowest ground point <br />within the parcel. <br /> <br />. Data to substantiate the BFE. These data <br />may be taken from an authoritative <br />source, such as the COE, USGS, SCS, any <br />other Federal agency; State and local <br />water resource or planning <br />departments, a FEMA FIS, or information <br />prepared by a registered professional <br />engineer. <br /> <br />S. A signed statement asserting the <br />accuracy of the information submitted <br /> <br />General Technical Guidance <br /> <br />When developing technical supporting data, the <br />requestor should consider the following points: <br /> <br />. If an entire legally described parcel of <br />land is shown to be inadvertently <br />included in the SFHA, the LOMA will be <br />issued for the parcel. <br /> <br />. If the request is for more than one lot or <br />structure, the requestor must provide an <br />effective FIRM or FHBM, or a <br />photographic copy of it, on which the <br />legal metes and bounds of the property, <br />including each lot, have been drawn as <br />accurately as possible. The map <br />submitted must bear the seal of a <br />registered professional engineer or <br />licensed land surveyor to certify that the <br />property is correctly located on the NFIP <br />map based on the legal description of <br />the property. <br /> <br />If a floodplain boundary runs through a <br />lot or tract of property, the location(s) of <br />the structure(s) on the lot or tract should <br />be accurately drawn. A photocopy of <br />the NFIP map is unacceptable because <br />the size of the map may be distorted. <br /> <br />. In some instances, FEMA may need <br />additional data to make a <br />determination. These data may consist <br />of, but are not limited to, certifications <br />by a registered professional engineer or <br />licensed land surveyor as to the type of <br />structure and whether it is elevated on <br />posts, piers, pilings, or frontal sand <br />dunes, or a hydraulic analysis of the <br />flooding conditions. <br /> <br />. To support a request involving fill that <br />was placed before the effective date of <br />the first NFIP map showing the structure <br />or parcel of land to be within the SFHA, a <br />requestor may be required to submit a <br /> <br />36 <br />
