COVERAGE UNDER THIS PERMIT (coni.) <br /> <br />permittee shall submit a completed \Ietice of Transfet and Acceptance of Terms of a Construction General <br />Stormwater Dischargt; Permit Cenific,;,tion that is signed in accordance with Pan I.E. I. of this permit. The Notice <br />of Transfet form is available from the Division and includes: <br /> <br />a) Permit cenification numb,r. <br />b) Name, location, and coumy for the construction site for which the application is being submitted. <br />c) The current permittee's name. address, telephone number and the status as Federal, State, private, public Ot <br />other emity. <br />d) The new permil1ee's nam!:, addt"55 and telephone numbet and the status as Fedetal, State, ptivate, public or <br />othet emity. <br />e) Cenification tha: the new perrllinee has reviewed the permit and SWMP and accepts responsibility, coverage <br />and liability for the permi:. <br />f) Effective date of ttansfer. <br /> <br />7. Pennit Exoiration Date <br /> <br />Authorization to discharge unde! this general permit shall expire on June 30, 2002. The Division must evaluate and <br />reissue this genetal permit at lea,t one" every five yeats and must recenify the applicant's authority to discharge <br />undet the general petmit at such tirre. Many permittees will have filed a Notice of Inactivation (see section A.5. <br />above) by that time and will not be af~'"ted by permit expiration. Any permittee desiring continued coverage undet <br />the general permit must reapply by Match 31, 2002. The Division will determine if the applicant may continue to <br />operate undet the terms of the gener~" permit. An individual permit will be tequired fOt any facility not <br />teauthorized to discharge under the tei,;sued general permit. <br /> <br />8. Individual Penni! Criteria <br /> <br />Aside from the activity type, the Divisi,)n may use owet criteria in evaluating whether an individual permit is <br />required instead of this general permit. This infonnation may come from the application, SWMP, or additional <br />information as tequested by the Divisiol1, and includes, but is not limited to, the following: <br /> <br />a) the quality of the receiving water!; (i.e., the ptesence of downstteam drinking water intakes or a high quality <br />fishery, or for preservation of hi,;h quality water); <br />b) the size of the construction site; <br />c) evidence of noncompliance undet a previous permit for the operation; <br />d) we use of chemicals within the stonnwatet system. <br /> <br />In addition, an individual permit may b,'; required when the Division has shown Ot has teason to suspect that the <br />stormwater discharge lIIay contribute to a violation of a water quality standard, <br /> <br />B. CONTENTS OF THE STORMWATER MANAGEMENT PLAN <br /> <br />The SWMP shall be prepared in accordanct with good engineering, hydrologic and pollution control practices. (The <br />SWMP need not be prepared by a regis.tered engineer.) The main objective of the plan shall be to identify Best <br />Management Practices (BMPs.) which when inplemented will meet the terms and conditions of this permit (see Pan I.c., <br />below). <br /> <br />The plan shall identify potenllal sources of pollution (including sediment) which may ;,easonably be expected to affect the <br />qualiry of stOnTIwater discharges associated W.th construction activity ftom the facility. In addition, the plan shall descrihe <br />and ensure the implementation of BMP:; whic1 will be used to teduce the pollutants in stOnTIwarer discharges associated <br />with construction activity. Construction op,ef:'ltions must implement the provisions of the SWMP required under this parr <br />as a condition of this pennil. <br />
