A. COVERAGE UNDER THIS PERMIT (coni.)

permittee shall submit a completed \Ietice of Transfet and Acceptance of Terms of a Construction General
Stormwater Dischargt; Permit Cenific,;,tion that is signed in accordance with Pan I.E. I. of this permit. The Notice
of Transfet form is available from the Division and includes:

a) Permit cenification numb,r.
b) Name, location, and coumy for the construction site for which the application is being submitted.
c) The current permittee's name. address, telephone number and the status as Federal, State, private, public Ot
other emity.
d) The new permil1ee's nam!:, addt"55 and telephone numbet and the status as Fedetal, State, ptivate, public or
othet emity.
e) Cenification tha: the new perrllinee has reviewed the permit and SWMP and accepts responsibility, coverage
and liability for the permi:.
f) Effective date of ttansfer.

7. Pennit Exoiration Date

Authorization to discharge unde! this general permit shall expire on June 30, 2002. The Division must evaluate and
reissue this genetal permit at lea,t one" every five yeats and must recenify the applicant's authority to discharge
undet the general petmit at such tirre. Many permittees will have filed a Notice of Inactivation (see section A.5.
above) by that time and will not be af~'"ted by permit expiration. Any permittee desiring continued coverage undet
the general permit must reapply by Match 31, 2002. The Division will determine if the applicant may continue to
operate undet the terms of the gener~" permit. An individual permit will be tequired fOt any facility not
teauthorized to discharge under the tei,;sued general permit.

8. Individual Penni! Criteria

Aside from the activity type, the Divisi,)n may use owet criteria in evaluating whether an individual permit is
required instead of this general permit. This infonnation may come from the application, SWMP, or additional
information as tequested by the Divisiol1, and includes, but is not limited to, the following:

a) the quality of the receiving water!; (i.e., the ptesence of downstteam drinking water intakes or a high quality
fishery, or for preservation of hi,;h quality water);
b) the size of the construction site;
c) evidence of noncompliance undet a previous permit for the operation;
d) we use of chemicals within the stonnwatet system.

In addition, an individual permit may b,'; required when the Division has shown Ot has teason to suspect that the
stormwater discharge lIIay contribute to a violation of a water quality standard,

B. CONTENTS OF THE STORMWATER MANAGEMENT PLAN

The SWMP shall be prepared in accordanct with good engineering, hydrologic and pollution control practices. (The
SWMP need not be prepared by a regis.tered engineer.) The main objective of the plan shall be to identify Best
Management Practices (BMPs.) which when inplemented will meet the terms and conditions of this permit (see Pan I.c.,
below).

The plan shall identify potenllal sources of pollution (including sediment) which may ;,easonably be expected to affect the
qualiry of stOnTIwater discharges associated W.th construction activity ftom the facility. In addition, the plan shall descrihe
and ensure the implementation of BMP:; whic1 will be used to teduce the pollutants in stOnTIwarer discharges associated
with construction activity. Construction op,ef:'ltions must implement the provisions of the SWMP required under this parr
as a condition of this pennil.
