(sewer line in home) (walls) <br />(floors)? <br />3. Do you have flood insurance? (Yes) (No) (if yes policy #) <br />4. Do you have (a basement) (crawl space) (no basement) <br />5. What kind of damage did you incur? give written estimate.... <br />6. If a low interest loan is available (up to $2500) are you <br />interested? <br />7. If so, how much would you need? <br />8, Did the Red Cross survey your property for damage? If so, do <br />you know your claim number? _, . <br /> <br />Release: <br /> <br />I hereby release the above information to the Town of Frederick <br />(Firestone) and state and local disaster planners will be used <br />in evaluating my urgent unmet needs. <br /> <br />Signed <br /> <br />The results of the survey will be tabulated into a data base for <br />informational purposes. Towns will declare a local disaster on Thursday, September <br />27, 1990. <br /> <br />2. Mayor Taglente will assemble newspaper articles, photos, and <br />other materials pertaining to the flood event and mail to <br />Kistner at DODES. <br /> <br />3. The towns' of Frederick and Firestone will mail out the <br />questionnaire this week. This will be valuable information <br />for use in our background material. <br />
