<br />3 <br /> <br />Upon completion of the project, your community may request that we make a final determination on <br />revising the effective PIS report, FIRM, and FBFM by submitting the data listed below. <br /> <br />. As-built plans, certified by a registered professional engineer, of all proposed project <br />elements <br /> <br />. If the as-built conditions differ from those shown on the previously referenced construction <br />plans, a hydraulic analysis, for as-built conditions of the base flood and floodway, as well <br />as for the floods having a 10-, 2-, or O.2-percent chance of being equaled or exceeded in <br />any given year, must be submitted, accompanied by a topographic work map showing the <br />revised floodplain and floodway boundaries. Any revised analyses or work maps must be <br />certified by ~ registered professional engineer. <br />, <br /> <br />. A community map reflecting the City's updated corporate limits <br /> <br />. Community acknowledgment in the form of a letter from the Chief Executive Officers <br />(CEOs) of both the City of Loveland and Larimer County, or officials designated by the <br />CEOs, stating that the communities have reviewed and concur with the request for a fmal <br />determination on revising the effective FIRM, FBFM, and FIS report and will adopt and <br />enforce the modified floodway <br /> <br />. A copy of the public notices distributed by your community and Larimer County stating <br />the communities' intent to revise the floodway, or a statement from each community that <br />it has notified all affected property owners <br /> <br />. Copies of the letters from your community and Larimer County to the Colorado Water <br />Conservation Board notifying it of the floodway modification <br /> <br />. Certification that all fill placed in the currently effective base floodplain and below the <br />proposed BFE is compacted to 95 percent of the maximum density obtainable with the <br />Standard Proctor Test method issued by the American Society for Testing and Materials <br />(ASTM Standard D-698) or an acceptable equivalent method for all areas to be removed <br />from the base floodplain <br /> <br />. Effective October 1, 1996, FEMA revised the fee schedule for reviewing and processing <br />requests for modifications to published flood information and maps. Under this schedule, <br />FEMA established flat review and processing fees for most types of requests. All requests <br />dated October 1, 1996, or later will be processed under this new schedule. The fee for <br />your map revision request will be $2,300 and must be received before we can begin <br />processing your request. Payment of this fee shall be made in the form of a check or <br />money order, made payable in U.S. funds to the National Flood Insurance Pro~ram, or <br />by credit card.
