It involves <br />taking those actions necessary to prevent <br />suffering and hardship or to remove <br />threats to safety of life or property. Such <br />work is temporary in scope and intended <br />to provide immediate and limited service <br />to the political subdivision. <br /> <br />Permanent Work <br /> <br />Permanent work is directed toward the <br />restoration or replacement of a facility or <br />system based upon pre-disaster design. <br />Other considerations are: current codes, <br />specifications and standards required to <br />not only comply with state and local build- <br />ing regulations but also to mitigate against <br />the possibility of a similar occurrence <br />during a future emergency or disaster. <br /> <br />It Must be Noted that the Local <br />Disaster Declaration - Request for <br />Assistance - Comes Between Damage <br />Assessment and the Following MateriaL <br /> <br />Damage Survey Report <br />(DSR) <br /> <br />The Damage Survey Report (DSR) is a <br />detailed description of damage with rela- <br />tivel y exact cost estimates for and to <br />repair, replace or restore each damaged <br />facility or system to its pre-disaster condi- <br />tion (see Appendix C). For example: Road <br />shoulder damage for 1/4 mile, 3' wide and <br />3' deep; road bed washed out in two <br />places, length 50 yards and 125 yards, <br />average depth 4', and two cnlverts <br />destroyed, 18"x30" and 24"x30'. Itemized <br />list of materi.als, equipment and labor to- <br /> <br />tals $5,560. The DSR serves as a basis for <br />performing eligible work and. obtaining of <br />federal and state funds for such work. The <br />applicant is responsible for assisting the <br />team in the following: <br /> <br />Completing a Notice of Interest Form <br />identifying those categories of damage <br />to be examined (see Appendix F). <br /> <br />Identifying potentially eligible disaster <br />damage and leading the team to those <br />locations. <br /> <br />Plotting all damage sites on ap- <br />propriate maps . <br /> <br />Providing all information pertinent to <br />the damage site, i.e., insurance and <br />work performed to date. <br /> <br />The preparation of a DSR is per- <br />formed by a DSR team consisting of <br />the various levels of government in- <br />volved. The team chief will be either a <br />federal or state inspector assigned to <br />the team. <br /> <br />][neligible Damage Sites <br /> <br />Damage sites can be declared ineligible if: <br /> <br />The site has less than the minimum <br />dollar amount of damage to qualify the <br />site for federal or state assistance. <br /> <br />For other reasons. <br /> <br />If the applicant disagrees with in- <br />eligibility recommendation, reasons for <br />disagreement should be written on <br />separate piece of paper and attached <br />to the DSR. <br /> <br />A DSR should be completed for every <br />eligible site and submitted to the FEMA <br />Public Assistance Officer for review. <br />Resurveying or additional information for <br /> <br />Eleven <br />
