DUNS CONTRACTOR ESTABLISHMENT NUMBER: The Offeror shall in the <br />space provided on Standard Form 19-B, Page 4, insert his assigned nine (9) <br />digit Dun and Bradstreet, Inc., (DUNS) Contractor Establishment Number. <br /> <br />21. EQUIPMENT OWNERSHIP AND OPERATING EXPENSE SCHEDULE: Whenever a <br />modification or equitable adjustment of contract price is required, the <br />Contractor's cost proposals for equi pment ownership and operating expenses <br />shall be determined in accordance with the requirements of paragraph: <br />EQUIPMENT OWNERSHIP AND OPERATING EXPENSE SCHEDULE, contained in the Special <br />Provisions section of the specifications. A copy of EP 1l10-1-8 <br />"Construction Equi pment Ownership and Operating Expense Schedule" is <br />available for review in this office and in the office listed in paragraph: <br />SITE INSPECTION herein. <br /> <br />22. COLORADO SALES AND USE TAX. Specific exemption from the Colorado <br />Sales and Use Taxes will be granted by Colorado tax authorities with respect <br />to all materials used by a Prime Contractor or subcontractor and which are <br />built into structures furnished under contract to a Government agency. The <br />Colorado sales and use taxes shall be excluded from bid prices.
