<br />. <br /> <br />. <br /> <br />. <br /> <br />39 <br /> <br />Quarterly Reports <br /> <br />~ :""5: <br /> <br />How to Manage a Planning Grant Award <br /> <br />Local jurisdictions awarded an FMA Planning Grant A ward are required <br />to submit quarterly progress reports according to the calendar year as <br />follows: <br /> <br />Quarter Endinf <br /> <br />Report Due Bv <br /> <br />March 31 <br />June 30 <br />September 30 <br />December 31 <br /> <br />April 15 <br />July 15 <br />October 15 <br />January 15 <br /> <br />Appendix 15 contains the quarterly report format that must be used. A <br />copy of the report is also located on the computer diskette. <br /> <br />The local jurisdiction must indicate on the quarterly report any problems, <br />delays, or adverse conditions wbich will materially impair the ability to <br />meet the objective of the FMA planning grant award. Any favorable <br />developments which will enable meeting time schedules and objectives <br />sooner Of at a lower cost than anticipated or producing more beneficial <br />results than originally planned should also be highlighted on the <br />quarterly report. At a minimum, the quarterly report must include: <br /> <br />. Local point of contact name, title, address, phone number, and <br />facsimile number <br /> <br />. <br /> <br />Start date of project (i.e. grant award date) <br /> <br />. <br /> <br />Anticipated close-out date <br /> <br />. <br /> <br />Project status (e.g. on schedule, behind schedule etc.) <br /> <br />. <br /> <br />Percent of project complete (e.g. 25% complete) <br /> <br />. <br /> <br />Project cost status (e.g. cost underrun) <br /> <br />. <br /> <br />Accounting summary including grant award, funds requested, and <br />funds spent <br /> <br />. <br /> <br />Additional comments <br /> <br />Mail complete quarterly reports to: <br /> <br />Missouri State Emergency Management Agency <br />A TTN: State Hazard Mitigation Officer <br />POBox 116 <br />Jefferson City, Missouri 65102 <br /> <br />How to Manage a Planning Grant Award <br />
