<br />10.B.(3) A final construction report containing the following <br />information, if applicable in accordance with the requirements of Rule <br />6: <br /> <br />10.B.(3)(a) A summary of construction, problems <br />encountered, and solutions implemented to resolve the problems; and, <br /> <br />10.B.(3)(b) A summary of construction material tests <br />and geologic observations; and, <br /> <br />10.B.(3)(c) Photographs of construction if required, <br />from exposure of the foundation to completion of construction; and, <br /> <br />10.B.(4) A record of the location of permanent monuments and <br />instrumentation as well as initial surveys and readings shall be <br />submitted if applicable. <br /> <br />10.B.(5) A schedule for the first filling of the reservoir, <br />specifying fill rates, water level elevations to be held for <br />observation, and a schedule for inspecting and monitoring the dam. No <br />filling schedule is required for minor dams rated Class III and all <br />Class IV dams or if waived by the State Engineer for good cuase <br />shown. The owner, however, shall monitor the dam frequently during <br />the first filling. <br /> <br />10.B.(6) A long-term instrumentation monitoring plan for new <br />dams and enlargements (except for minor Class III and all Class IV <br />dams) which shall include: <br /> <br />10.B.(6)(a) The frequency of monitoring; <br /> <br />10.B.(6)(b) The data recording format; <br /> <br />10.B.(6)(c) A graphical presentation of data; and, <br /> <br />10.B.(6)(d) The parties who will perform the work. <br /> <br />10.B.(7) The engineer shall provide periodic review of the <br />data included in the long-term monitoring plan on at least an annual <br />basis for the first five years, whereupon the monitoring shall <br />continue in accordance with Rule 15.C.(2) and 15.C.(3). The engineer <br />shall submit the data and a written assessment of the dam's <br />performance to the State Engineer annually. <br /> <br />10.B.(8) An Emergency Preparedness Plan which conforms to <br /> <br />Rule 16. <br /> <br />10.B.(9) Upon written request by the owner and for good <br />cause shown, the State Engineer may temporarily approve storage of <br />water prior to full compliance with Rule lOB. The written request <br />shall include a schedule for compliance with Rule lOB. <br /> <br />- 36 - <br />
