<br />6.A.(2)(a) A completed application form provided by the State <br />Engineer shall be submitted, accompanied by a plan for the repair or <br />alteration and the appropriate specifications, which were prepared by <br />an engineer. The provisions-of Rules5.A.(9), 5,A.(10) (cost estimate <br />and fees) and Rule II.B. (nonjurisdictional dams) shall apply. <br /> <br />6.A.(2)(b) The plans and specifications shall contain <br />sufficient detail to enable a contractor to bid on, and construct the <br />repair, or alter the dam. The provisions of Rule 9 (construction <br />quality control), except for the time limits of Rules 9.A.(I), <br />9.B.(I), and 9.C.(I), and Rule 10 (acceptance of construction) shall <br />apply. The engineer shall give as much notice of the start of con- <br />struction as possible; and <br /> <br />6.A.(2)(c) Upon completion of a repair, the engineer shall <br />file AS-CONSTRUCTED plans that are in conformsance with Rules 5.A.(2) <br />and 5.A. (3). (See Rule I1.B.) <br /> <br />6.A.(2)(d) AS-CONSTRUCTED plans are not required for altera- <br />tions to nonjurisdictional size. <br /> <br />6.A.(3) Plans for the repair of a small or minor size Class III <br />dam, or any Class IV dam; and plans for the alteration of any Class <br />III or Class IV dam. to nonjurisdictional size are exempt from the <br />provisions of Rules 5A, 6, 9 AND 10 except as specified in Rule <br />6.A. (3)(a). <br /> <br />6.A.(3)(a) The dam owner must provide at least thirty days <br />advanced written notice to the State Engineer, The written notice <br />must contain the name of the dam, the location of the dam, the name of <br />the owner, and a clear description of the work to be performed. If <br />the State Engineer determines that plans and specifications prepared <br />by an engineer are necessary for the repair, the owner will be <br />notified within five working days of the receipt of the notice. The <br />owner cannot begin construction until the plans and specifications are <br />approved by the State Engineer. If plans and specifications are not <br />required. the State Engineer will inform the dam owner of engineering <br />and construction requirements, if any, and will perform construction <br />inspections as he deems necessary. The dam owner must keep the State <br />Engineer informed of the project status, and provide the State <br />Engineer with "AS-CONSTRUCTED" drawings and specifications within <br />sixty days following completion of the work. The "AS-CONSTRUCTED" <br />drawings must be drawn on good quality paper with permanent ink (or <br />equivalent), such that the drawings are reproducible, and suitable as <br />a long lasting permanent record <br /> <br />-.27 - <br />
