<br />A) Contact information including (Name, mailing address, phone, fax and email) of the <br />proposed grantee, and information regarding all interested parties and affiliates ofthe <br />grantee. <br /> <br />. <br /> <br />B) A detailed explanation of the proposed purpose and use(s) of the easement. <br /> <br />C) A site-survey, by a licensed surveyor, of the property covered by the proposed easernent. <br /> <br />D) A copy of the confirmation letter notifying the following parties of the intent to apply for an <br />easement: the Corps of Engineers-Om~a District, the Urban Drainage and Flood Control <br />District, South Suburban Parks and Recreation District and the CWCB-Flood Protection <br />Section. <br /> <br />E) A demonstrated awareness of and understanding! of the pertinent environmental regulations <br />and required floodplain permits. <br /> <br />, <br />F) For all drainage easement requests, a site drainage study and plan must be completed. <br /> <br />G) Easement requests within the 100-year flOOdplain of the Chatfield Downstream Channel <br />Improvement Project must include a hydraulic analysis to show impacts on the flood <br />conveyance for the Proj ect. <br /> <br />H) A written statement demonstrating awareness of and compliance with the applicable local, <br />state and federal environmental and floodplain regulations. <br /> <br />. <br /> <br />6. Any and all easements granted shall be temporary in nature and revocable for a maximurn term <br />limit of 25 years. <br /> <br />7. The CWCB may charge fees for easements and has the right to amend the fees or fee structure, in <br />accordance with the CWCB fee structure for project easements that the CWCB approved on March <br />13, 1996. See attached document for Easement Fee structure. <br />I <br /> <br />8. In determining whether to grant an easement and in determining value, The CWCB may consider <br />the following factors, among other considerations: . . <br /> <br />A. Whether the requesting party is a: <br />1) Governmental body, <br />2) Private governed utility, or <br />3) Private entity. <br /> <br />B. Whether the area under, upon, or over which ~e easement is requested is: <br />1) An undeveloped area that will never to be (leveloped, <br />2) An undeveloped area held primarily for future development, <br />3) Developed area with minimal usage, Or <br />4) Developed area with heavy usage. <br /> <br />C. Whether use of the easement by the requesting party would: <br />1) Terminate, restrict or require a change in the use of property, or <br /> <br />. <br />
