<br />5.3.4 The general conditions shall include statements that the plans and specifications <br />cannot be significantly changed without the prior written approval ofthe State Engineer in <br />accordance with Rule 9.1.8. <br /> <br />5.3.5 The general conditions shall include the provision that construction shall not be <br />considered complete until the State Engineer has accepted the construction in writing. <br /> <br />5.3.6 The specifications shall provide that the owner's engineer will monitor the quality of <br />construction as specified in Rule 9. If the engineer monitoring the construction for the <br />owner is not the engineer who developed the design and construction documents, the <br />engineer monitoring the construction for the owner is responsible for the quality of <br />construction, compliance with the approved design and specifications, and preparation of <br />the necessary documentation for the State Engineer's review and approval of all <br />construction change orders, and preparation of the project completion documents required <br />in Rule 10. <br /> <br />5.3.7 The specifications shall include as a minimum, but are not limited to, the following: <br /> <br />1. The quality of materials used in construction; <br /> <br />2. The acceptable quality of workmanship; <br /> <br />3. The reference to applicable standards, if any; <br /> <br />4. The required tests and estimated frequency of testing; and, <br /> <br />5. The action to be taken if unsatisfactory materials or workmanship are <br />discovered in the construction. <br /> <br />5.4 Hazard Classification Report - The hazard classification report shall identify the size and <br />hazard classification category for the proposed dam, or enlarged existing dam. A report is not <br />required for dams that are declared as High Hazard; however, a dam failure inundation map will be <br />required for the Emergency Action Plan pursuant to Rule 16. The report shall include sufficient <br />information regarding assumptions, calculations and data used to develop the dam failure flood <br />hydrograph and an assessment of the impact of the dam failure upon the downstream floodplain. <br />The dam shall be classified according to the definitions of Rule 4. The hazard classification report <br />shall be in a form acceptable to the State Engineer and shall include, but not be limited to: <br /> <br />5.4.1 Dam failure inundation maps are required for all dams classified as High Hazard and <br />Significant Hazard. Inundation maps are required for dams classified as Low Hazard unless <br />the dam is located in a remote area where no development exists downstream ofthe dam; <br /> <br />5.4.2 Cross-sections along the watercourse, drawn to scale, showing water surface <br />elevations at critical locations where structures may be impacted by the flood wave. Cross- <br />sections shall show discharge in cubic feet per second, average velocity in feet per second, <br /> <br />12 <br />
