A schedule of the daily rates of the firm's personnel <br /> <br /> <br />effective for the duration of the study, together with <br /> <br /> <br />automo~ile and four-wheel-drive vehicle costs per mile, <br /> <br /> <br />per diem travel rates, etc. <br /> <br />6. The name of each principal and lead professional for <br /> <br /> <br />each major discipline expected to work on the study, the <br /> <br /> <br />academic training and professional experience of each <br /> <br /> <br />such person, and the component of the study for which <br /> <br /> <br />each person will be responsible. Please specify who the <br /> <br /> <br />study manager would be. <br /> <br />7. The firm's past experience in conducting studies similar <br /> <br /> <br />to this one. Please include at least three references <br /> <br /> <br />of entities for which the firm has recently performed <br /> <br /> <br />similar work (include the names of persons who can be <br /> <br /> <br />contacted at these entities). Also, please indicate the <br /> <br /> <br />firm's experience in handling studies where there were <br /> <br /> <br />public involvement activities.
