<br />B, A monthly summary report indicating at a minimwn the specific dates, start and end <br />times, total time, and weather conditions when each hail cannon was operated, <br /> <br />C, An annual report shall be submitted to the Director no later than December 31 of each <br />year for the preceding operations, The annual report shall include a complete record of <br />operations, a nwnerical evaluation of the effectiveness of the project in the target areas, <br />any impacts in the area reasonably expected to be effected, and any recommended <br />changes in the operating plan. <br /> <br />All such reports shall be public records subject to the provisions and limitations of Part 2 of <br />Article 72 ofTitle 24, CRS, <br /> <br />The annual report shall be submitted in a timely manner to the following entities: <br /> <br />-A: Director,-Colorado WaterCons-ervation Board ~ <br />B, Colorado Division of Wildlife District Office <br />C, Saguache County Board of Commissioners <br />D, AJamosa County Board of Commissioners <br />E. Town Council of the Town of Center <br />F, Hon, Lewis H. Entz <br /> <br />I I. The State reserves the right to examine and audit the daily logs, books, and records of the <br />Permittee concerning the project which is the subject of this permit for a period of two <br />years after the termination date of this permit. <br /> <br />12. The Permittee shall be responsible for obtaining all permits, licenses, or other approvals, <br />and for filing all notices or reports, required by federal, state, or local laws in addition to <br />this permit. <br /> <br />13. The annual commercial fee required by section 36-20-113, CRS, shall be two percent of <br />the cost for each year's operation and shall be paid to the Director by December 3 I of each <br />year, <br /> <br />14, All notices, reports, records, payments and other communications that the Permittee must <br />provide to the Director shall be addressed as follows: <br /> <br />Director <br />Colorado Water Conservation Board <br />1313 Sherman Street, Room 721 <br />Denver, Colorado 80203 <br /> <br />Telephone: 303-866-3441 <br />FAJ(: 303-866-4474 <br /> <br />14, Violations of or deviations from the "Terms and Conditions" of this permit may result in <br />revocation or suspension ofthis permit as provided by law, <br /> <br />4 <br />
