<br />4. Customer purchases an approved ULV from dealer and obtains a <br />receipt of purchase. Toilet must use no more than j.6 gallons of <br />water per flush. <br /> <br />5. Customer informs Arvada Utilities Division that they have purchased <br />a UL V toilet. <br /> <br />6. An Arvada representative/inspector goes to the customer's home or <br />structure where toilet is installed to ensure proof of purchase and <br />that an approved toilet is installed. A valid sales receipt or proof of <br />purchase should show the date, place of purchase, make and model, <br />and actual volume per flush. <br /> <br />7. Arvada will mail a check to the customer as soon as possible. <br /> <br />8. When a customer accepts the $80 rebate, the customer agrees to an <br />interview and/or questionnaire regarding their level of satisfaction <br />with the UL V toilet within a year after installation of the UL V toilet. <br /> <br />9. The toilet removed must be disabled. <br /> <br />.
