<br />, . <br /> <br />\ <br /> <br />Page 4 <br /> <br />(3) Evaluating PermIt ApplIcatIons <br /> <br />Information provided on Form WM2 will be used by the ExecutIve <br />D1rector to decide (based on the criterja in C.R.S. 5 36-20-112) <br />if a permit should be granted. <br /> <br />(4) Granting of Permjts <br /> <br />Permits shall be granted for a maximum of one year. Exceptions <br />may be made for projects scheduled to last up to 18 months. Per- <br />m1ts may be renewed during the five years following the date of <br />issue prov1djng conditions under which the permjt was 1ssued have <br />not changed substantially. <br /> <br />C. Reports <br /> <br />(1) A current, daily log (whjch includes the information requ1red by <br />Form WM3) shall be kept by the permittee at the project office and <br />made available for inspection by the public in a manner that does <br />not interfere with the project. <br /> <br />(2) Form WM3 shall be submitted biweekly to the Executive Director not <br />later than ten days after the end of the reporting period. <br /> <br />(3) Annual reports shall be made in accordance with C.R.S. <br />5 36-20-117(3). A written final report whjch evaluates the <br />project shall be subm1tted to the Executive Director and to county <br />commission offices (in those counties in which publication of a <br />legal notice of intent is required) within 30 days of completion <br />of the project. <br /> <br /> <br />2 CCR 401-1 <br />
