• EXHIBIT B <br /> Method/Procedure <br /> Construction methods and procedures shall be in accordance with the final signed and sealed engineering <br /> project plans and specifications. All operations shall be performed following OSHA and all other local, <br /> State and Federal laws and regulations. <br /> Deliverable <br /> Signed and dated paper and electronic copies of As-Built drawings prepared by engineer of record on 11" <br /> x 17"sheet size. <br /> C. REPORTING AND FINAL DELIVERABLE <br /> 1. Designs and Changes Reporting: <br /> - CWCB shall provide to the CDPHE Project Officer a copy of the Spillway Design showing the PE and <br /> SEO stamps. <br /> - CWCB shall provide copies of all change orders,design supplements and other design or construction <br /> documents to the CDPHE Project Officer. <br /> - CWCB shall provide a copy of the As Built drawings upon project completion. <br /> 2. Monthly Reporting: <br /> - CWCB shall submit a monthly narrative that details the project progress. These narrative reports shall <br /> continue until the project reaches completion, even if funding from CDPHE has been exhausted. The <br /> narrative shall include percentage completed and a summary of the monthly activities. <br /> - The narrative report shall describe the completion or partial completion of the tasks identified in the <br /> statement of work including a description of any major issues that have occurred and any corrective <br /> action taken to address these issues. <br /> - CWCB shall include any reports concerning construction oversight in the monthly narrative report. <br /> - CWCB shall provide in the narrative report a budget report that includes"local match"and percentages <br /> expended of all funding sources. <br /> 3. Final Deliverable: <br /> At completion of the project,the CWCB shall provide to CDPHE a final report that summarizes the <br /> project and documents how the project was completed. This report may contain photographs,summaries <br /> of meetings and engineering reports/designs. <br /> D. BUDGET <br /> The total project cost is estimated to be$4,500,000. Approved funding for thel project includes the <br /> $1,500,000 WSRA grant($75,000 from the Rio Grande Basin and$1,425,000 from the statewide <br /> account),a CWCB loan in the amount of$1,000,000.The Summitville Natur l Resource Damage <br /> account will provide$2,000,000 of project costs,$1,000,000 each from the State and Federal NRD <br /> Funds.The federal NRD funds have already been transferred and are being he}d by CDPHE for <br /> disbursement. <br /> Page2of4 <br /> Exhibit B-Statement of Work <br /> V 1/MAR2012 <br />
