each firm and individual. The consultant shall also identify its teams members <br />that will be working on each of the tasks within the scope of services. <br />9. Qualifications <br />The Consultant should briefly provide information showing the qualifications of <br />the Consultant to conduct this work. Qualifications are to be given for both the <br />firm as well as individuals selected to perform the work. Qualifications shall also <br />be provided for any subcontractors that would perform work under this proposal. <br />10. Conflicts of Interest <br />The Consultant should identify any actual or perceived conflicts of interests <br />relative to the firms or individuals that would be involved in the work effort, the <br />basis for those conflicts, the potential consequence of those conflicts, and how the <br />Consultant proposes to handle those conflicts. <br />11. Costs <br />The Consultant is to provide the cost proposal in a summary table in separate <br />sealed envelopes. The cost information will not be opened for review until after <br />the proposals have been reviewed and a Consultant selected or a "short list" of <br />potential Consultants selected for further consideration. Cost information shall be <br />provided in a format similar to that shown on the Attached Exhibit C and shall be <br />broken down for each Work Task identified in Exhibit B, and ideally should be <br />broken down further into subtasks to permit State representatives and other <br />reviewers to adequately analyze the proposal and its costs. Cost information <br />contained in the proposal may be considered in making the final selection, and <br />will serve as a basis from which the States will negotiate the final terms of the <br />contract. <br />In the sealed cost envelopes, the Consultant shall also provide their hourly rate <br />and reimbursable expense schedules. It is anticipated that the contract will be <br />based on hourly rates and reimbursable expenses with a maximum contract <br />amount that cannot be exceeded without prior written consent of the State <br />representatives. <br />If the Consultant is proposing scope alterations, as per item 6 above, additional <br />cost proposals should be provided to address the effects of the scope alterations <br />on the proposed cost. These additional cost proposals should be in the same <br />format as the cost proposal addressing the Work Tasks in Exhibit B to facilitate <br />cost comparisons by those evaluating the proposals. <br />-5- <br />
