We wi}1 <br />initiate the Federal and state coordination required to determSne if the r�nains warrant a recovery effort <br />or if the site is eligible for listing in the National Register of Historic Places. <br />4. If you sell the property associated with this permit. you must obtain the signature ofi the new owner in <br />the space provided and forward a copy of the permit to this office to validate the transfer of this <br />authorization. <br />5. If a conditioned water quality certification has been issued for your project, you must comply with the <br />conditions specified in the certification as special conditions to this permit. For your convenience, a <br />copy �f the certification is attached if it contains such conditions. <br />6. You must allow representatives from this office to inspect the authorized adivity at any time deemed <br />necessary to ensure that it is being or has been accomplished in accordance with the terms and conditions of <br />your permit. <br />Special Conditions: <br />1. The permittee shall implement the mStfgation plan described in the Department of the Arny permit <br />applfcati� prepared by Pioneer Enviroraner�tal Services. Incorporated, dated Apri1 14, 2004, inclusive of <br />pages 17 through 27 and figures 6, 6a. 6b. 6c. 6d and 6e. However, the monitoring period for all wetland <br />mitigation sftes is extended to five (5) years for all sites and an additional five (5) years for the Delta <br />Area, or until the Corps determines successful mitigation. <br />2. The permittee shall provide as-built drawings that provide elevations 9n plarrview and Cross-section of <br />the mitigation sites identif9ed in Fi.gures 6, 6a, 6b. 6c. 6d and 6e prior to pro�ect construction. <br />3. The permittee shall augment the mitigation with a description of vegetative treatmer�ts which includes <br />revegetation methodotogy, final species selection, seed mixes, spacing of woody plants and herbaceous plarrt <br />plugs and the source of off-site plant materials prior to profect construction. <br />4. A mitigation work implementation schedule shall be provided to the Corps for approval prior to <br />construction activities at Elkhead Dam and Reservoir that are within the jurisdicti�n ot the Corps. <br />5. The permittee shall prepare a monthly construction inspection report describing work compTeted to date <br />and any modifications needed to resolve problems encountered. The monthly report will address impacts to <br />wetlands and status of mitigation. <br />6. The permittee shall identify the entity responsible for miti gation and monitoring activities prior to <br />construction, and after implementation of the mitigation plan. The permittee shall submit comprehensive <br />annual mitigation monitoring reports. and arrange with the Corps for annual onsite review of wetland <br />mitigatian areas. <br />7. The permittee shall perform all other mitigative measures identified in the Department of the Army <br />permit apptication, dated Apri1 15. 2004. <br />8. The permittee shall comply with the Fish and Wildlife Service (FWS) Final Programnatic Biologica} <br />Upinion on the Management Plan for Endangered Fishes in the Yampa River Basin, dated January 10. 2005. <br />9. The permittee shall comply with all conditions contained in the attached pro�ect specific FWS Final <br />Biologtcal Opinion number ES/GJ-6-LO-04-F-012-YPDO1, dated February 9, 2005_ <br />10. The permittee shall comply with all conditions contained in the attached State of Colorado. Section 401 <br />Water Quality Certification number 4018, dated August 2. 2D04. <br />
